As far as the received message at that time I was envisioning the plus
addresses showing up in Sender:, Envelope-to:, and Return-Path:.

No, since then I've done a little research.  Several MTAs rewrite
Sender:, and the othe two headers are not reliably/widely supported.
Methinks we'll have to expose the consent token in the from:/To:
headers, embed it in the Message-ID, and potentially think about use of
the Subject header for control messages.
