Yes. It be called from within a milter and report its results before the
SMTP transaction has ended. Several milters doing so are posted at various
locations around the net.

But milters are new, complex, require deep understanding of sendmail and
c, and are several orders of magnitude more complicated and unreliable
than executing procmail as a local mail delivery agent. 

I fully expect several readers of this message to reply that they have
implemented calling procmail in a milter, and that it is easy to install.
Pre-empting such message, I should tell the other readers that the writer
of such a message intends to convey the information that he is a star.
Others may not find it so easy.
