When I read this first time, I feel some ambiguousness about
'and/or'. Just after that, I think it is obvious that in multicast
case, two sessions with different multicast address and same port
number should be distinguished, because of existence of 'or'. Is this
correct?

If so, I think it is better to separate the sentence to point-to-point
case and multicast case, for smooth understanding. Ex.

For point-to-point unicast case, the multiple RTP sessions are
distinguished by different port number pairs. For multicast case, the
multiple RTP sessions are distinguished by different port number pairs
or different multicast addresses, or both of them.

Just my thought...
