[AVT] Update (-05) on Timed Text

Hi all,

I just submitted an update to the draft.  This doc adresses the comments
before and during the IETF.

In this update I kept the video/ registration because the issue is not
solved yet.

- responded to comments from AVT mailing list
- fragmentation: pointed out that, although not mandatory, modifiers should
split at meaningful boundaries to reduce effects of packet loss
- included reference to RTP regarding timestamp resolution and RTCP
measurements and synchronization
- examples in section 3 revised
- removed static SIDX start value of 129
- revised offer and answer section
- O/A examples
- new MIME registration. Two parameters added to express capabilities
- editorials/clarifications.

Pending Issues

- should this payload format register timed text under the 'text' top-level
type, under 'video' or both?
