[bmwg] hash-stuffing 04 change log

hash-stuffing 04 has two changes, both minor.

1. The ID never explicitly stated that Ethernet test traffic should use
random rather than static addresses. It does now, in section 4.2:

"Test traffic SHOULD use pseudorandom patterns in Ethernet addresses."

2. Also in section 4.2, we added a recommendation that port number IDs be
1-indexed rather than 0-indexed to avoid the small but nonzero probability
of producing an all-0s MAC address.

Since we've received no outside input since draft 02, we urge that draft 04
be moved to WGLC.
