I replied to Mark earlier describing the 639 data in a way that isn't quite accurate wrt 639-3: in the main, normative, data file, every ID has an associated reference name, and those are required to be unique. There is also an informative Language Names Index file that lists alternate names for each ID (including the reference name); these names are not guaranteed to be unique. So, the reality for 639-3 isn't exactly as you describe it, but probably close enough.
