I have submitted a revised version taking most of the input into
consideration. See Change Log for the major parts.

A remaining issue is how to include a code for NETLMM case. It is more
complicated than I thought since it depends on what assumptions we can make.
Is the MN running FMIP even if not running MIP? Anyway, we can address it in
the next version.
