This new revision reflects the WGLC comments.  Basically the document
has been much simplified.

Changes:

- Removed pre/post-authorization stuff

- Removed description on IP address update

- Removed authorization and accounting considerations section.

- Removed context binding description.

- Added clarification that EAP authentication is performed over PAR
and PAN exchanges.

- Added backward compatibility section

- Editorial fixes.
