There are is a remailer system that seeks to prevent traffic analysis,
the very thing that pen registers do:

I see no reason why (for a modest degradation in performance) similar
techniques couldn't be applied to voice. These systems are nearly
unique in that their security is dependent on the number of users of
the system.
