[Sip] Update to REFER

I've just submitted

to the I-D repository.

This revision incorporates feedback from the IESG. The changes
were additions to the security considerations to discuss replay
and cut-and-paste issues with the use of message/sipfrag, and 
a more detailed discussion of the risks associated with acting
on a REFER request, particularly when the reference is to a 
non-SIP URI.

The updated sections are 5.2 and 5.3.
