[TLS] Review of draft-ietf-tls-rfc4366-bis-01

This draft seems to mostly OK, but I think it still
needs some changes.

- The hash in client_certificate_url needs to be mandatory.
  IIRC, that was WG consensus when this was discussed.

- In S 4, I wonder if too long packets MUST be rejected
  rather than may.
